Standard Plan

Three options are available at a flat rate of $10,000, $25,000 or $50,000.

Enhanced Plan

• Employee and spouse - $10,000 to $100,000 in units of $5,000**
• Each child - flat $3,000

**If Enhanced coverage is purchased, the combined Standard and Optional
Maximum Principal Sum amount cannot exceed $100,000 per Insured.
