Short-Term Disability:
- Up to 75% of salary to maximum of $2,000 per week
- Benefit periods from 30 days to 2 years
- Own occupation definition
- 0 day hospital and accident and 7 day sickness elimination period
- Maximum age 70
Long-Term Disability:
- Up to 70% of salary to maximum of $10,000 per month
- Two year Own occupation definition followed by any occupation definition
- 30, 90 or 180 day elimination period options
- Maximum age 65
Permanent Total Disability:
- Lump sum payment of up to 5 times annual salary
- Maximum lump sum benefit $500,000
- 12 or 24 month elimination periods
- Maximum age 65
